Adding New Fields - The website cannot display the page

I installed 1.3.3 (com_ckforms_b5.zip) and cannot add any fields to my form. When I click on "new", I am receving an HTTP 500 Internal Server Error..The website cannot display the page. Any thoughts? I do see the new tables in phpmyadmin after install..

Here is the actual error:

Is this an issue with my php setup?

I am using Joomla 1.5.15, php 5.2.9-2, and MySQL server version 5.1.40

I guess you just need to have a php server to check if what could be the most possible error to the page. About the form, maybe you should check if you got some error on the scripts. It could be one thing to see if there is an error to it.
