Free text like fieldset

Hello all,

I would like to know if it's possible to add free text with CkForms.

I just want to make 2 parts in a same form, like :

Subtitle1:
Name : Input
Email : Input

Subtitle 2:
Tel: Input
Fax: Input

I've made several tests without success in particular with separator field.
