I am getting this error when trying to view forms data in an article

CKForms Error Message :
Form 'BamCall[start:0,' not found !!!

I have been unable to find anything on this, please help.

Also, what is "start record index" as shown below?

Replace the characters set “formname” with the “Ckform” form name you want to display the data.
The followings parameters are optionals :
start is the start record index
number is the number of records displayed
showborder display the table border (value : 0 or 1)
showheader display the table header (value : 0 or 1)
shownavigation display
