Header whites-out/blanks out when using form

Hello,

Everything seems to work great, but when I embed the form onto a page, the header of the page is whited out/blanked out. The page I am trying to place this on is here: http://www.vanticore.com/about-us/contact-us . The images within the slider at the top gets whited out. Tried embedding in an article as well as a component. Any ideas?
