how do I make a title/question for each field

Hi:

This is very new to me; I can't figure out how to make a title for each field. For example, I want to say 'Have you tried our new product?' and then a yes and no radial button. but I can't figure out how to get the question in there. All is see is field name and I can't seem to put it in there as it won't take spaces. Where do you put the field question? Thank you.

To have a title for each field, you have to put your text in the field "Label"
