identify the form page

Hi,

is it posibble to identify the article page, from which was the form send?
It would be nice in a situation, when you want to use the same form on different pages, and need to identify the page.

For example you want to use it as a simple order form, when you have article to describe some product and another article to describe another product, and you want to place the same form on both pages, but need to know, which product is the user ordering.

It would be nice to be able to place form to the article with some additional parameters, that will than be send to the administrator and saved to a database.

I hope it is clear...
