Form Module problem

I have created a form with just two fields and enabled the Save results option under the settings.

This form I need to show as a module. So in the CK form module I select this form and it shows . On submission, the form sends a mail, but the data is not saved in the database and the form data is empty.
Even the other forms alos are not saving the data although the option is enabled.
The component version is CKForms V1.3.4 and there is only one module version. Joomla version is 1.5.18.

It is very urgent. Pl. suggest a solution

I have done the same thing... put my form into a module.
It opens ok and it saves the data... but I don't receive the email... I have done everything I read in this forun and don't work.

Can you help me?
