Cant read data . Using Ckform as article

I have created a general form and im displaying it in a article. I've got three small errors. Hope some one can help me out.

I am able to enter the data submit the form



But i cant see any data in the backend



Can someone please guide bout where I'm doing wrong ??

An unwanted string is appearing on the form.I have no idea how . The screenshot below
