Reuse Form

I have a form I designed for one client that, with minimal adaptation, I can reuse on another clients site.

What do I need to copy to reuse it? Copy the CSS I have altered (I hacked the CSS to change input box sizes and a bunch more) obviously. But I'm guessing some of the info is actually stored in the MySQL database? Am I right?
