Userdata in Form

At this moment I can only get name, username and e-mail in a form

I would like to expand with other data ( which is available via Community Builder) eg address, zipcode, city, telephone, etc

How to extend te selectionlist with other userdetails???

I have exactly the same request.
I want to use informations that are already in the Community Builder profile of users.
Is it possible and how to do (even with php modifications...)
