Uploaded docs not attached and multi tick box option?

Hi
Have installed CK Forms and all works okay with forms sent and emails being sent to senders and recipients containing entered content but the problem is the uploaded documents are not being attached to the emails or being placed in specified folders online. Have checked the folder path is correct but still content or attachments to emails...any ideas anyone?

Also any idea how I can have more than one tick box per question?
