Multi Checkbox, comments and rating

I wish insert in a form a "multi checkbox" and no many single checkbox, is it possible?

Also, how can I attach to a form a system of rating and comments (I use JComments)?
The idea is that user list frontpage, open a form and then append comments or rating the form.
So, rating and number of comments are reported into frontpage list.
