Drop down with effect

Hi

I want to create a drop down list that when you choose one it displays another field or two in the form?

E.g.
If I have a list of Names with Cell phone numbers and Addresses.

You would choose the Name from the list and when you choose someone's name, the Cell Phone Number and Address appears either next or under.

Is this possible?
